Vitamin D and testosterone: the study everyone quotes, and the one that followed it
One small 2011 trial made vitamin D look like a hormone fix. A better-designed trial and a pooled analysis of eight studies reached a different conclusion.
If you read comments under any article about hormones, vitamin D comes up within a few replies, usually with a screenshot of a testosterone number that supposedly doubled after supplementing. The claim traces back to a real study. It also has a follow-up that almost nobody mentions.
The study that started it
In 2011, a group in Austria and Germany reported a subgroup analysis of an earlier randomised trial in overweight subjects. Men who took vitamin D showed higher serum testosterone afterwards, and the authors were appropriately cautious: they had shown an association in a small subgroup, and whether a causal link existed was unknown.
That caveat did not survive the internet. The finding became "vitamin D raises testosterone", and a whole category of men started supplementing on the strength of it.
The trial built to answer the question
Eight years later the same research group published the study that the 2011 paper really called for. The Graz Vitamin D and Testosterone trial was double-blind, placebo-controlled, and randomised, and it deliberately enrolled the men most likely to respond: one hundred healthy men who had both low total testosterone and low vitamin D levels at the start. Half took 20,000 IU of vitamin D3 weekly for twelve weeks; half took a placebo. Testosterone was measured by mass spectrometry, the more accurate method.
The result was null. No significant treatment effect on total testosterone, and none on the secondary measures — free testosterone, the free androgen index, SHBG, estradiol, LH or FSH. The authors' conclusion was direct: vitamin D treatment had no effect on testosterone levels in these men.
That is not a fringe finding contradicting an established one. It is the same group, testing their own hypothesis properly, and the hypothesis did not hold.
What the pooled evidence says
A 2019 systematic review and meta-analysis pulled together every randomised controlled trial of vitamin D supplementation in men it could find — eight studies, ten effect sizes — with daily dose equivalents ranging from 600 to 4,000 IU to 60,000 IU weekly, and durations from six weeks to three years.
Across all of them, vitamin D had no significant effect on total testosterone or on SHBG. Subgroup analyses by dose, frequency, and starting levels also showed no significant effect. The reviewers found no evidence of a hormonal benefit and asked for larger trials, which is the polite way of saying the current evidence is thin and points nowhere useful.
More recent pooled analyses of randomised trials have continued in the same direction.
So is vitamin D useless?
No — this is the part that gets lost when a claim collapses. Vitamin D has well-established roles in bone health and calcium balance, and genuine deficiency is common, especially in people who work indoors, wear covering clothing, or live at high latitudes. Treating a diagnosed deficiency is sensible medicine. What is not supported is the idea that supplementing will move your testosterone.
The practical difference is real. If you take vitamin D expecting a hormone change, you will be disappointed and may have over-supplemented for no reason. If you take it because a blood test showed you are low, you are doing something reasonable for your bones.
What to do instead of guessing
- Test, don't assume. A 25-hydroxyvitamin D blood test is cheap and widely available, and it is the only way to know whether you are actually low.
- Treat a deficiency as the goal, not a number from a forum. Where the target sits is a clinical decision, not a crowd-sourced one.
- Be wary of "more is better". Fat-soluble vitamins accumulate, and very high intake over long periods carries its own risks. Doses belong in a conversation with someone who can see your bloods.
- Don't let a supplement crowd out the boring things. If vitamin D is the most sophisticated thing in your routine, the routine is the problem.
The honest summary: vitamin D is a reasonable thing to correct if you are deficient, and a poor thing to expect a hormone result from. The trial designed to test the hope came back empty, and the pooled evidence agrees.
