Herbal testosterone supplements: what the trials actually found

A systematic review examined the herbs sold as testosterone boosters. Two had reasonable supporting trials, several had almost nothing, and the reasons matter.

Walk into any supplement shop and the testosterone shelf is mostly plants: fenugreek, ashwagandha, tribulus, tongkat ali, maca, ginseng. The marketing is interchangeable. The evidence behind each one is not, and the differences are large enough to be worth knowing before you spend anything.

The most useful place to start is a 2021 systematic review in Advances in Nutrition that went looking specifically at herbs and testosterone concentrations in men, rather than at one product at a time.

Where there are real randomised trials

Fenugreek has the most human data, largely because a few standardised seed extracts have been tested commercially. In a double-blind randomised placebo-controlled trial of 120 healthy men aged 43 to 70, twelve weeks of a standardised extract at 600 mg per day was associated with lower scores on an androgen-deficiency symptom questionnaire, improvements in reported sexual function, and increases in both total and free testosterone compared with placebo.

That is a properly designed trial with a positive result, and it should be read with two qualifications. It tested one specific commercial extract at one dose in middle-aged and older men, and a positive symptom questionnaire is a softer outcome than a hormone measurement. Later work has continued to test fenugreek preparations — a 2024 placebo-controlled trial published in PLOS One measured testosterone in both plasma and saliva — and the picture across preparations is not uniform. Different extract, different dose, different result is the normal state of affairs in this literature.

Ashwagandha (Withania somnifera) has several randomised placebo-controlled trials, mostly small. In one eight-week trial of fifty men reporting low sexual desire, 300 mg of root extract twice daily was associated with significantly better sexual-function scores and a significant change in serum testosterone relative to placebo. Its other trials tend to report benefits on stress and sleep measures alongside modest hormonal changes — which, given how much stress and sleep matter to testosterone on their own, makes the mechanism genuinely hard to untangle.

Where the marketing runs far ahead of the data

Tribulus terrestris is the clearest example of a product whose popularity has no proportionate evidence behind it. A review of phytotherapeutics in this area found the human data on testosterone unconvincing; despite decades on shelves as a testosterone booster, controlled trials in men have generally not shown the hormonal effect the label implies. Its reputation rests heavily on animal work and on studies that measured something other than testosterone in men.

Tongkat ali, maca and ginseng each have a handful of small human studies, often with mixed outcomes, varied preparations, short durations, and endpoints that shift between hormones, symptoms and semen parameters. The systematic review's overall verdict on the field was not "these work" — it was that the evidence is limited and inconsistent, and that better trials are needed.

The three problems that undermine most of these results

Preparation. "Fenugreek" is not one ingredient. A standardised extract tested in a trial and a cheap seed powder in a proprietary blend are different products, and a positive trial on the first says nothing about the second.

Baseline. Trials that enrol men with low testosterone, poor sleep, high stress or low sexual desire have far more room to show improvement than trials in healthy men with normal levels. Several of the positive results come from exactly those populations.

Proprietary blends. Most products on sale contain a dozen ingredients at undisclosed doses, none of them matching a tested protocol. Whatever a trial found, the product in your hand was probably not the thing tested.

How to read a testosterone booster label

Look for the ingredient, the specific extract, and the dose per serving. Compare it against the trial. If the label hides amounts behind "proprietary blend", there is no comparison to make — that is the point of the blend.

And treat the two categories differently. Fenugreek and ashwagandha are the two with randomised placebo-controlled trials worth reading, and even there the effects reported are modest and preparation-specific. Tribulus is a product with a marketing history rather than a supporting evidence base.

The honest summary

No herb in this literature does anything comparable to fixing your sleep or losing excess body fat, and the review authors were explicit that the evidence remains limited and inconsistent. If you want to try fenugreek or ashwagandha, that is a defensible choice based on real trials, provided you buy the tested extract at the tested dose and expect a small effect. If a product's pitch is louder than its evidence, that gap is the whole product.
